Medicare

Medicare has not established the coverage policy for this test. For Medicare patients, an Advance Beneficiary Notice of Noncoverage (ABN) is required before testing can begin. Because Medicare will not cover test costs until a reimbursement policy has been established, test costs will be billed to the patient. A summary of instructions and documentation requirements for Medicare patients is included on page 1 of the ABN.

Health Insurance Portability and Accountability Act (HIPAA)/Privacy Practices

Myriad Genetic Laboratories, Inc., is dedicated to offering high-quality laboratory services in the area of predictive medicine and is committed to ensuring patient privacy through compliance with HIPAA and applicable state privacy statutes and regulations.

HIPAA, enacted by Congress in 1996, required the Department of Health and Human Services to establish national standards for electronic healthcare transactions and national identifiers for providers, health plans, and employers. In addition, HIPAA also contains provisions that address the security and privacy of health data and information.

Myriad Genetic Laboratories, Inc., has established and maintains an active privacy program that ensures patient confidentiality and is fully compliant with the HIPAA Privacy Rule. As mandated by the Privacy Rule, Myriad Genetic Laboratories, Inc., requires that all employees undergo privacy awareness training and has implemented administrative and technical safeguards that are focused on protecting patient privacy. Myriad is also committed to compliance with the HIPAA Security and the Transactions and Code Sets regulations prior to their respective compliance deadlines. Various states have enacted or issued regulations regarding patient privacy. It is the policy of Myriad Genetic Laboratories, Inc., that test results are disclosed only to the requesting healthcare provider/designee and are not disclosed to anyone else without the written permission of the patient.
